Course Evaluation Survey
This is a course evaluation form to be filled at the end of the semester
which collects feedback about the instructor and the course. In
addition to the form view, a summary is provided. PSW version provides
save feature which allows students to fill the form online. All saved
forms will be available to the file owner.
