Employee Evaluation Form

This is a survey for evaluating employee performances. 5 scale rating system is used. The rate titles are editable. PSW version provides save feature. Hence, the survey may be filled online and the data on the sheet may be saved by the Employee name. A collection of evaluation forms may be created using the online form.
